Child Enrollment Application
This is the Enrollment Application that is required by Pinellas County Florida and Accelerated Learning Centers of America for each child to attend. Please Download, Complete, and have available at your child's or children initial interview.
School Entry Exam
This file is to be filled out by the Parent and Health Care Provider. We request that this Document is completed and presented during your child's or children initial interview.
Emergency Medical Release Form
This form must contain only one child's name, and be the original notarized form. A new Notarized Form is required when there is change in legal guardianship. Please Download, Print, Complete, and have available at your child's or children initial interview.
Child Health and Development Questionnaire

Food Experience Permission Form
